Hospital-acquired conditions (HACs) are a major contributor to hospital-based morbidity, mortality, and cost. However, multifaceted changes in organizational and systemic factors that promote or hinder quality improvement (QI) are needed to prevent and manage HACs. The purpose of this project was to test the effectiveness of a QI approach that emphasizes multimodal strategies to reduce HACs. A QI initiative was implemented in a tertiary academic medical center in northern Georgia. This initiative was implemented using a basic ecological framework to enhance the understanding of why HACs occur and how to reduce them. A plan for implementing systems-wide improvements in patient flow, infection prevention, and environmental cleanliness was developed. To test the hypothesis, we measured change in process and outcome measures before and after implementation. Thirty percent (43 of 150) patients had a HAC after admission for inpatient care at our institution. A pre-post intervention design was used to assess measures of process and outcome of care in a cohort of patients. After the intervention, there was a significant reduction in HACs (P =.02). The reduction in HACs was greatest for measures of infection prevention processes and environmental cleanliness. This study demonstrates the multifaceted approach of a QI project that resulted in an 11.9% reduction in HACs in a tertiary academic medical center. Additional educational and administrative interventions may result in further reductions in HACs and maintenance of these changes over time.The ultimate goal of the Cell and Molecular Imaging Core (CMIC) is to provide investigators with cost effective, high quality image analysis solutions that are relevant to their research. This will be accomplished by providing access to sophisticated equipment (imaging, image analysis, and microscopy) and application know-how, to interested researchers.
